Patient Care

I consent to an Elite Wound Care staff member to enter my home and give me a physical evaluation. I understand that a physical consists of assessing vitals, gathering height and weight, evaluation of open wounds, and physically touching the areas in which the patient is having issues with.

I understand that Elite Wound Care staff may need to use the facilities, access safe water, and utilize the patient's method of disposal.

I understand that if Elite Wound Care staff see any signs of children or elderly abuse, it is our duty to report it to the correct authorities. I understand that if there are any drastic medical anomalies, that 911 will be called if that is required to save the life of a patient (such as, but not limited to, anaphylactic reactions, heart attack, seizure, etc).

Staff Care

I am responsible for paying the towing bill of any staff members vehicles towed in my living facility if I did not disclose to staff ahead of time the rules and stipulations of my living facility in writing. (If applicable) This does not apply to public streets or where there are visible warning signs.

I understand that sexual harassments or verbal abuse is not an appropriate form of conduct. I understand that if an Elite Wound Care Staff member feels uncomfortable for any reason, they can leave at any point. We do not tolerate discrimination of any kind and encourage reporting of any staff members who do not abide by those rules.
